Live Scan machines are either operated by the California Department of Social Services, through a contract with Identix, or by the California Department of Justice through law enforcement offices and other state-government sites (i.e., school districts), or private entities. When you have your fingerprints scanned using Live Scan technology, you should:

1. Complete the TrustLine Registry Application Form/Live Scan Form (TLR2), dated 10/05.

2. Call Identix at 1-800-315-4507 to obtain the location nearest to you and to schedule a time to get your fingerprints scanned.

• Tell the Identix operator that your application will include payment for the full TrustLine fee.
• The Identix operators will ask you to provide the information from the TrustLine Application/Live Scan form so that the information will be in the computer when you arrive at your appointment.
• Take the completed Request for TrustLine Registry Application/Live Scan Service form and payment in the amount of $87.00 with you. Have the operator who scans your prints complete the box at the bottom of the form. This form will be the only receipt and evidence you have that your fingerprints have been submitted. The completed, signed TrustLine Application/Live Scan form needs to be sent along with a check in the amount of $43.00 (made payable to CA Department of Social Services) to the address in Box 10.
• Please be aware that the Identix operators only schedule fingerprint appointments. They do not have any information about TrustLine.

3. If there is not an Identix location near you, contact your local Police Department or Sheriff's Office and request an appointment to get your fingerprints scanned. Law enforcement will keep one copy of your TrustLine Registry Application/Live Scan form. So make sure to take two completed copies of the TrustLine Registry Application/Live Scan form with you to your fingerprinting appointment. Some law enforcement sites will require use of the LIC 9163 Request for Live Scan Service form, so take both the TrustLine Registry Application/Live Scan Service form as well as the Request for Live Scan Service form with you when you get fingerprinted. Make sure that on both the TrustLine application form, in the Live Scan section, and on the LIC 9163 form the Job Title is listed as "Employee" or "Emp". Law enforcement sites charge a $71.00 fee for the TrustLine background check. There will be a fingerprint rolling fee in addition to the $71.00 fee which varies from site to site and ranges between $10.00 and $25.00. Have the operator who scans your prints complete the box at the bottom of the form. This form will be the only receipt and evidence you have that your fingerprints have been submitted. The completed, signed TrustLine Application/Live Scan form needs to be sent along with a check in the amount of $43.00 (made payable to CA Department of Social Services) to the address in Box 10 of the TrustLine Application form.
